Attached please see a form that CMS has allowed us to share with providers concerning the first worksheet that is given out during the MDS Survey.  Also see a portion of that email that was sent to us.

We are aware that many of you are getting questions and perhaps pressure from providers or associations to release information about the MDS/Staffing Focused Surveys.  Please continue to direct them to email us at MDSforSandC@cms.hhs.gov and we’ll be happy to answer their questions promptly.

Finally, we understand that it can be helpful for facilities to be better prepared to provide surveyors with information when requested.  For example, the faster a facility provides a list of their residents with the requested information, the faster the survey team can begin conducting their reviews.  Worksheet #1 is handed to the facility staff upon entrance to be completed.  If state agencies would like to share this worksheet (#1) with the provider community outside of the survey, they may do so.
